While the doll enters little nightmare 3 is the only collectible found in any chapter of the game, and some chapters have secondary collectibles that are unique to that chapter. For Chapter 4: Lab, the secondary collectible is Ghosts.
The ghost in Chapter 4 is a ghost that can only be seen by approaching the area where the ghost resides. It disappears after watching a short animation, which counts as collecting the ghost. There are a total of 6 items to collect, and collecting them all will unlock the ‘Institutionalization’ achievement.
Chapter 4: Where to Find All the Ghosts in the Lab
As mentioned, You need to find a total of 6 ghostsAnd they will be spread out pretty well throughout the chapter. The first can be used before you actually start the chapter, and the last can be used right before the final battle with this chapter's boss.
If you replay Chapter 4 of Little Nightmares 3 to get the ghost you missed: You may not see any ghosts you collect on your first run.. That said, they still count toward unlocking the 'Institutionalization' achievement.

Doing so will cause a large foot to come out of the left wall, revealing a hole in the wall. Go left through the hole in the wall and into the back left corner.A bench sits next to a bundle of luggage. The third ghost will be sitting on this bench.Kicking your feet. |

Go through what looks like a bunch of mushrooms Find yourself at the bottom of a large staircase. Before going up the stairs, go to the back left of the room, near the clothes rack.Find the fourth ghost standing on the floor. |

While being chased by the boss for the second time, You will find yourself in another hall with a lever that Low must hold to open the door for Alone. She can then go inside and destroy the wall to let him through.. Once you complete this task, you can proceed immediately, but Before leaving the room where Alone was destroying the wall, go to the back right corner. Look for the fifth ghost sitting on a bench along the right wall. |

After taking a long elevator ride down to what appears to be a concrete basement.You will immediately exit into an area where you have to cross wooden planks with a locked door on the other side. The final ghost is on the floor next to the bed in the locked room.. To get the key, cross the planks and head back towards the elevator and you will see a hole in the right wall. Go through the hole to find the key In the box in the room. |
