Baldur's Gate 3 is hard enough, but if you want to make it even harder, use honor mode. This forces you to play with a single save file, and if your party is wiped you have to start over from the beginning.
Of course, this still means that team members can fall without consequence. It's okay if only one party member is alive, and you can resurrect everyone after the battle. So why don't we make it even harder?
Nexus Mods user NecroticFlame's Companion Permadeath does exactly what it says on the tin. If a companion falls into battle, they are gone forever. Yes, this means you can no longer be resurrected via items, spells, or wither. Other companions may also react to their deaths, and the character will no longer be involved in the story moment. They are gone forever.

“Without this mod, when a companion dies, there are several ways to resurrect them,” the mod description reads. “If you don't want to resurrect them, perhaps promising their death for roleplaying purposes, you can have the Withers 'protect' them, temporarily removing them from your party.
“But! Their corpses will still hang around awkwardly in the tent. Everyone will talk as if they are alive. And sometimes they are even resurrected by the game in certain cutscenes involving them. They rarely die. This mod changes everything. Now, your companions will leave the party as “dead” the moment they die.
It is noted that there is also a “Special Person with Story Resurrection” that will give you a unique second chance in the game with this mod installed. If I had to guess, this means that if Gale dies before Mystra stabilizes the sphere, there will be one more chance to resurrect Gale. That's because when she dies, Gale actually leaves a message telling anyone she encounters to perform a ritual to bring her back to life. Otherwise, the sphere will perish and everything around it will be destroyed.
But what about other people? It will probably quit as soon as its HP reaches 0. Or at least when he failed to save Death. This would be the perfect mod for a Nuzlocke run in Baldur's Gate 3, and could mean that you'll be able to take advantage of some of the recruitment that Withers provides. Or, if you're not in honor mode, you can do things like reload multiple times to make sure your favorites survive.
