There are a few things players should consider before using the Chronicle system.

Once an item is identified in the game in which it was found, it will be added to the Chronicle. Unidentified items are not counted until they are identified in the same session.

Like runewords, you must craft them by socketing them into the correct item to register them.

Chronicle tracking not only works across your accounts, it also maintains your progress across different types of sessions, including multiplayer, online, and offline modes. Separate offline session single player files are the only exception to the rule.

Any previous items acquired prior to the release of 3.0 (Reign of the Warlock) will not be considered for completing the Chronicle. There is no way around this other than using Stash's Share tab.

Set items should be the most difficult items to obtain because it is unknown how the game's RNG will react to the player's attempts to obtain them all.
However, some places are better than others to farm set items, and some players have a good win rate and get them right away.
